Building a bridge to continued success at home

Transitioning you from the hospital and through the various steps to recovery requires a holistic vision for your care and seamless transitions to ensure success from the hospital to your home. As a nonprofit organization, our vision for you has always been greater than just your rehabilitative stay. We are committed to advancing your progress by maximizing outcomes through rehabilitative therapy and skilled nursing and designing an individualized plan for success as you transition back home.

Get Out and Stay Out!

As mentioned above, our ultimate goal is to get you on the road to recovery as quickly as possible. We endearingly call it our “Get Out and Stay Out” program. What research shows us is that your recovery is not a quick fix. We will lay out a plan for success but it will require your determination as well. We want to help you prepare with pre-surgical conditioning, enhance the recovery you started in the hospital into rehab, prepare for outpatient therapy once you leave campus, and then design a fitness maintenance program to help you keep all the progress you have worked so hard to achieve.

Pre-Surgical Conditioning

If you or a loved one is planning a surgery and looking to improve your post-recovery outcome, our certified fitness trainers will work with you to improve cardiovascular fitness, strength, and physical stamina to achieve the highest result post-surgery.

Outpatient Rehab

Outpatient therapies are available in our on-site Therapy Center to any patient under Medicare Part B or with a physician’s order.

Fitness Center

Our fitness center is open to our residents, clients, and the community. We have specialized equipment that is gentle on joints and easy to operate and also offer memberships, one-on-one training, and a variety of fitness classes to meet your unique needs including: arthritis, COPD, Congestive Heart Failure, and Diabetes

Our unique strength extends beyond the rehabilitative time in the long term care setting. We are becoming nationally recognized for our Take2 home care service, a model where clients receive the care they need to be successful and safe at home when they need it (i.e., ADL care, medication management, home health nurse, errand running, meals). Take2 reduces hospital re-admissions, is affordable, and can be delivered to the client’s own home or in independent or assisted living. Our commitment to your loved one’s hospital transition goes above and beyond to ensure they get back home as quickly, successfully, (and safely) as possible.
